How We May Use Your Health Information

We may use and disclose your health information for the following purposes:

Uses Requiring Your Authorization

Most uses and disclosures of your health information for marketing purposes, as well as disclosures that constitute the sale of your health information, require your written authorization. Additionally, psychotherapy notes (if applicable) and certain other sensitive information require specific authorization. You may revoke any authorization in writing at any time, except to the extent we have already relied upon it.

Our staff is trained to handle all patient information with the utmost confidentiality.

Filing a Complaint

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
